Erin Athey 

DNP, FNP-BC, FAANP
Founder and CEO

Family Nurse Practitioner

(she/her)

Dr. Erin Athey, a board-certified family nurse practitioner, has dedicated nearly two decades to improving health outcomes in Washington, D.C.'s underserved communities as a clinician, educator, and scholar. Her work has led to innovative, community-based health initiatives, including award-winning mental health literacy programs and a mobile health program for COVID-19 testing, vaccinations, and education across Wards 5-8. Driven to enhance healthcare access, Dr. Athey founded C3 Cares, a groundbreaking model featured in The Washington Post, ABC News, and Forbes, recognized for transforming healthcare delivery. She also serves as an adjunct faculty member at George Washington University and specializes in HIV/AIDS care.

Tracie Bass

Operations Coordinator

(she/her)

Tracie Bass has spent her career creating opportunities, building programs, and connecting people to resources that improve lives. Her professional journey began in Graduate Medical Education at The George Washington University, where she served for 15 years as Program Administrator for the Internal Medicine Residency Program. In that role, she supported the training and development of future physicians while ensuring excellence in medical education. Following her work in graduate medical education, Tracie joined The Rodham Institute at its founding and spent the next 12 years serving as Director of the Health Education and Leadership Program. She developed and led a wide range of educational and workforce development initiatives for learners at every stage—from middle school students exploring healthcare careers to medical students, residents, and practicing professionals. A common thread throughout her work has been a commitment to addressing health disparities and creating practical pathways to improve health equity in underserved communities. Her efforts expanded from George Washington University to collaborative work with Georgetown University and numerous community partners throughout the region. At the same time, Tracie served as an external consultant to the White House Medical Unit for more than 12 years, contributing her expertise in program coordination, operations, and healthcare. Today, Tracie serves as Operations Coordinator at C3Cares, where she is excited to return to the direct service space that has always been at the heart of her work. She is passionate about supporting individuals and communities that often face the greatest barriers to care and believes that meaningful change happens when compassion, education, and action come together. Throughout her career, she has remained dedicated to helping people reach their full potential while creating stronger, healthier communities for all.

Ebony Thomas

 RN, BSN

Nurse Manager

Ebony Thomas, RN, BSN is a Registered Nurse who has been serving the DMV area for over a decade, with experience in patient advocacy, clinical operations, and healthcare project management. She currently serves as a Nurse Manager with Community Concierge Care (C3 Care), supporting care coordination and program development to improve access and continuity of care. Her background includes leading immunization and emergency response initiatives, overseeing clinical operations and EMR implementations, and supporting federal health programs to deliver timely, high-quality care. She is currently completing herPsychiatric Mental Health Nurse Practitioner (PMHNP) and Doctor of Nursing Practice (DNP)programs and remains committed to strengthening healthcare systems through compassionate, patient-centered care.

Teria Sands

Teria Sands, a board-certified nurse practitioner, has over eight years of experience in acute, critical, and home health care. A D.C. native, she earned her BSN from Bowie State and MSN from Marymount University in 2024, now pursuing her doctorate. Passionate about public and maternal health, she joined C3 Cares in 2025 to help reduce health disparities. Teria values patient advocacy, compassionate care, and building strong connections. ​In her free time, she enjoys the outdoors, yoga, cooking, and time with loved ones.

Angela Carter, FNP-C, is the wife of a Marine Corps Officer and a proud mother of two beautiful daughters, Erin and Briana. A distinguished graduate of the University of Pittsburgh, Angela received her Bachelor of Science Degree in Nursing in 1987. Subsequently, she earned a Master of Arts from Webster University and a Master of Science from Duke University. Angela was the recipient of the Clinical Practice Excellence Award from Duke University in 1990 and has since practiced as a Family Nurse Practitioner. Additional career highlights include work as a Health Care Provider with a non-profit organization committed to providing quality health care and promoting wellness to patients in need. In 2001, Angela accepted a government position at Womack Army Medical Center to serve the health-care needs of both active and retired service members and their families. Angela’s passion is helping patients achieve optimal health and weight loss goals. iWith respect to managing work and life, Angela states, “It is challenging, exciting, and not without sacrifice, but I love it, and most of all God has been good to our Family.

Jameshia Waller

Registered Nurse

Jameshia Waller is a new graduate registered nurse with 12 years of healthcare experience as a Licensed Practical Nurse. She holds a Bachelor's degree in Psychology and recently earned her Bachelor of Science in Nursing from George Washington University School of Nursing. Throughout her career, she has worked in a variety of healthcare settings, developing a strong foundation in patient care and community health. Jameshia is passionate about improving access to quality healthcare and bridging gaps in care for rural and underserved populations through education, advocacy, and community outreach.

Oluwatimilehin “Timi” Ugbade, FNP, CRNP, is a dedicated Family Nurse Practitioner committed to providing compassionate, patient-centered care. She takes the time to understand each patient's unique needs, emphasizing preventive care, education, and personalized treatment plans that support long-term health and wellness.

Sifon Cole

Community Health MA
BS, CCMA

Sifon Cole is a Community Health Medical Assistant at C3Cares in Washington, D.C., with over 11 years of experience in healthcare supporting diverse patient populations through compassionate, client-centered care. She holds a Bachelor’s degree in Health and Wellness and is currently completing her Master’s in Clinical Mental Health Counseling at Walden University, and is completing her counseling internship. Sifon’s work bridges physical health and behavioral health, drawing on her background in medical assisting, care coordination, and patient advocacy. She is passionate about helping individuals feel heard, supported, and empowered in their care journeys. Her long-term goal is to support clients in healing mind, body, and spirit through integrative approaches to counseling that include cognitive-behavioral, creative, and holistic practices. Sifon believes that with the right support, people have the ability to reconnect with their strengths and actively participate in their own healing and well-being.

Jasmine is a dedicated Medical Assistant at C3, bringing over a decade of experience in the healthcare industry. She has worked as a Certified Clinical Medical Assistant (CCMA) since 2011. In her role, she focuses on patient care, efficient patient flow, and delivering exceptional customer service while maintaining strong telephone etiquette. Jasmine is passionate about helping individuals thrive and is committed to providing a positive experience that prioritizes patient satisfaction.

Victoria Fields

Certified Medical Assistant

Victoria Fields has been a Medical Assistant/Phlebotomist for five years. She has experience in OB/GYN, Family Medicine, and with LabCorp. She is currently enrolled at UDC, pursuing her ASN in Nursing, and aspires to become an anesthesiologist. Victoria is passionate about caring for others and meeting their needs. Sifon’s work bridges physical health and behavioral health, drawing on her background in medical assisting, care coordination, and patient advocacy. She is passionate about helping individuals feel heard, supported, and empowered in their care journeys. Her long-term goal is to support clients in healing mind, body, and spirit through integrative approaches to counseling that include cognitive-behavioral, creative, and holistic practices. Ohara Madikoto

Ohara is a skilled and tech-savvy virtual assistant with five years of experience supporting individuals and clinicians. With a background as a certified nursing assistant and volunteer experience helping patients with daily activities, she brings strong communication and interpersonal skills to her role at C3 Cares. Her responsibilities include scheduling appointments, managing calendars, handling email correspondence, data entry, and providing customer support.

Rebecca Ntabane

Community Health Administrator

Rebecca Ntabane is a Community Health Administrator at C3 Cares, specializing in program coordination, patient support, and community engagement.  With a background in communication and administration, she streamlines patient-provider coordination, supports marketing efforts, and ensures efficient data management. While pursuing a BA in Communication Science, she applies her academic insights to her professional role. Outside work, Rebecca runs a wellness channel promoting positivity and healthy living, while exploring entrepreneurial ventures.

Breyana Hopkins

MPH Public Health / CCMA
Patient Engagement Strategist

Breyana serves as C3 Cares’s Patient Engagement Strategist, focusing on building meaningful connections with patients and ensuring their voices are at the center of care. With Bachelor's Degrees in Health Sciences (BS) and Theatre (BA), as well as a newly awarded Master of Public Health from George Washington University, she is passionate about improving the patient experience. She joined C3 Cares in 2025 to support the mission of providing accessible, compassionate, and high-quality healthcare. In her role, she enthusiastically works to improve clinic operations, processes, and communication tools to enhance holistic, whole-patient care and support. She loves learning more about community and individual experiences in medical or social settings and applying those lessons to practice. With her creative mind and people-centered spirit, she is sure to connect with our community, advance the C3 Cares mission, teach others in ways that resonate, and hopefully make them smile! Alongside her strategic work, she is also one of our medical assistants! In her free time, she enjoys watching the arts, dancing, and crafting.
