Erin Athey 

DNP, FNP-BC, FAANP
Founder and CEO

Family Nurse Practitioner

(she/her)

Dr. Erin Athey, a board-certified family nurse practitioner, has dedicated nearly two decades to improving health outcomes in Washington, D.C.'s underserved communities as a clinician, educator, and scholar. Her work has led to innovative, community-based health initiatives, including award-winning mental health literacy programs and a mobile health program for COVID-19 testing, vaccinations, and education across Wards 5-8. Driven to enhance healthcare access, Dr. Athey founded C3 Cares, a groundbreaking model featured in The Washington Post, ABC News, and Forbes, recognized for transforming healthcare delivery. She also serves as an adjunct faculty member at George Washington University and specializes in HIV/AIDS care.

Pamela Slaven-Lee

DNP, ARNP, FAANP
Chief Operating Officer

Family Nurse Practitioner

Prior to joining C3Cares, Pamela Slaven-Lee, DNP, APRN, FAANP, served as the Provost and Vice President of Academic Affairs Marymount University in Arlington, Virginia. Her principal areas of clinical practice include leadership, administration, internal medicine, pathology, gastroenterology, hepatology, and simulation-based learning. Prior to joining Marymount, Dr. Slaven-Lee, served as both the interim dean and the senior associate dean for academic affairs at the George Washington University School of Nursing (GW). She also previously held faculty and leadership appointments at the Uniformed Services University and Georgetown University School of Nursing and Health Studies. Dr. Slaven-Lee is a fellow in the American Association of Nurse Practitioners and a Sigma Theta Tau Experienced Nurse Leadership Academy scholar. She holds undergraduate and graduate degrees in nursing from the University of Maine, and she earned a doctorate in nursing practice from Case Western Reserve University (CWRU). She was awarded the CWRU Frances Payne Bolton School of Nursing Young Alumni Award for Excellence in 2020. She is known for her work in graduate nursing simulation and experiential learning and has written extensively on the subject. Her work in the area of simulation-based learning has been nationally-recognized and disseminated as foundational in the design of competency-based simulation scenarios in advanced-practice nursing education. Her service has largely come in the provision of leadership at the curricular, program, school, and university level over the course of her career. Prior to her career in academe, Dr. Slaven-Lee practiced as a family nurse practitioner in Maine, providing care to under-resourced patient populations in rural areas. Her work has led to innovative, community-based health initiatives, including award-winning mental health literacy programs and a mobile health program for COVID-19 testing, vaccinations, and education across Wards 5-8. Driven to enhance healthcare access, Dr. Athey founded C3 Cares, a groundbreaking model featured in The Washington Post, ABC News, and Forbes, recognized for transforming healthcare delivery. Lisa Lindstrom

Chief Medical Officer

Lisa is proud to be part of a nurse-led clinic in community health.  She completed both her Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner degrees from George Washington University.  Her natural curiosity and care for people, the body, and the mind drew her to nursing in 2002.  Her passion for social justice and quality healthcare for all led her to C3 Cares. Her journey from chemist to dedicated nurse practitioner speaks volumes about her passion for connecting science with people.  She spends her free time enjoying the four generations of her family, cooking with her children, and traveling with her husband.

Teria Sands, a board-certified nurse practitioner, has over eight years of experience in acute, critical, and home health care. A D.C. native, she earned her BSN from Bowie State and MSN from Marymount University in 2024, now pursuing her doctorate. Passionate about public and maternal health, she joined C3 Cares in 2025 to help reduce health disparities. Teria values patient advocacy, compassionate care, and building strong connections. ​In her free time, she enjoys the outdoors, yoga, cooking, and time with loved ones.

Ashley Myers, FNP-C, is a Family Nurse Practitioner dedicated to caring for patients of all ages in the community. She focuses on building trust, listening closely, and providing care that supports the whole person. Passionate about prevention and wellness, Ashley works to make healthcare accessible, respectful, and empowering for every patient.

Bicki Serrano

Medical Assistant

Bicki Serrano, a skilled Medical Assistant with C3 Cares, lives and works in DC, focusing on expanding her knowledge in science, healthcare, and fitness. Originally from Honduras, Bicki is a native Spanish speaker who moved to the U.S. in 2018, driven by a desire to create a better life for her family back home. With support from her aunt, Bicki completed high school in the U.S. and continued her studies, earning her Medical Assistant certification in 2024. Alongside her medical work, she has enjoyed diverse experiences, from restaurant roles to enhancing her skills in education, language, and healthcare.

Khadijah Chase

Khadijah is a dedicated and compassionate CCMA with over 5 years of experience in different specialities. She lives and works in DC. She has experience working with patients who have chronic illness. She has strong empathy skills and always puts her best foot forward when taking care of patients' needs. Khadijah wants to continue her education and become an RN.

Ohara Madikoto

Ohara is a skilled and tech-savvy virtual assistant with five years of experience supporting individuals and clinicians. With a background as a certified nursing assistant and volunteer experience helping patients with daily activities, she brings strong communication and interpersonal skills to her role at C3 Cares. Her responsibilities include scheduling appointments, managing calendars, handling email correspondence, data entry, and providing customer support.

Daniel Davis

Community Outreach Specialist

(he/him)

For more than a decade, Daniel has dedicated his career to bridging the gap between underserved communities and the essential healthcare and social services they deserve. His work in community engagement began on the front lines at a men’s shelter, where he provided direct support and crisis intervention. This early experience gave him a deep understanding of the layered challenges vulnerable populations face every day. That foundation led Daniel to a specialized role as a Community Outreach Specialist with AmeriHealth Caritas DC, where he connects Medicaid members to preventive care, behavioral health resources, and critical plan benefits. He engages community members through culturally responsive education, builds trusted relationships with local clinics and faith-based organizations, and coordinates wellness events designed to improve health access and outcomes. Daniel is driven by a commitment to health equity, effective client navigation, and compassionate, trauma-informed communication. With a proven ability to build trust, de-escalate crises, and collaborate with interdisciplinary teams, he is seeking an opportunity to bring his skills to an organization focused on advancing community health and empowering underserved populations.

Jewel Adelsberg

Public Health Outreach Specialist

Jewel Adelsberg is a Public Health Outreach Specialist at Community Concierge Care (C3 Cares), where he leads community engagement, health education, and mobile clinics across multiple D.C. Housing Authority (DCHA) communities. His work prioritizes strong collaboration with DCHA, Managed Care Organizations (MCOs), academic partners, and community-based organizations to create seamless, accessible pathways to primary care for public housing residents. Additionally, Jewel is the primary staff member managing the Black Women’s Health Initiative pilot grant from DC Health to increase utilization of PrEP among Black Women in D.C.. He works closely with several key stakeholders to strengthen care coordination, improve communication systems, reduce administrative barriers, and align outreach with population health priorities. Jewel also contributes to C3 Cares’ program development and evaluation efforts, helping shape strategies that advance health equity across public housing. In 2023, Jewel earned his Bachelor’s in Public Health from the George Washington University Milken Institute School of Public Health, where he focused on community health, health policy and program evaluation. He is passionate about building trust, increasing care access, and supporting innovative, community-rooted models of health delivery.

Rebecca Ntabane

Community Health Administrator

Rebecca Ntabane is a Community Health Administrator at C3 Cares, specializing in program coordination, patient support, and community engagement.  With a background in communication and administration, she streamlines patient-provider coordination, supports marketing efforts, and ensures efficient data management. While pursuing a BA in Communication Science, she applies her academic insights to her professional role. Outside work, Rebecca runs a wellness channel promoting positivity and healthy living, while exploring entrepreneurial ventures.

Breyana Hopkins

MPH Public Health / CCMA
Patient Engagement Strategist

Breyana serves as C3 Cares’s Patient Engagement Strategist, focusing on building meaningful connections with patients and ensuring their voices are at the center of care. With Bachelor's Degrees in Health Sciences (BS) and Theatre (BA), as well as a newly awarded Master of Public Health from George Washington University, she is passionate about improving the patient experience. She joined C3 Cares in 2025 to support the mission of providing accessible, compassionate, and high-quality healthcare. In her role, she enthusiastically works to improve clinic operations, processes, and communication tools to enhance holistic, whole-patient care and support. She loves learning more about community and individual experiences in medical or social settings and applying those lessons to practice. With her creative mind and people-centered spirit, she is sure to connect with our community, advance the C3 Cares mission, teach others in ways that resonate, and hopefully make them smile! Alongside her strategic work, she is also one of our medical assistants! In her free time, she enjoys watching the arts, dancing, and crafting.
